Thank you for taking the time to participate in the Wellesley Health Department Community Health Needs Assessment.



This survey is designed to gather valuable information about the health and well-being of our community. Your input will help guide future programs, services, and resources that address the needs of our community. This survey is completely anonymous. Your responses will be kept confidential, and no personal identifying information will be collected. The results will be used solely for the purpose of improving health services and support in our community.



We want to hear from everyone—your experiences, concerns, and suggestions are important, regardless of your health status or background. The survey is 20 questions and should take about 10 minutes to complete, and your responses will make a meaningful difference in shaping community health initiatives.
