MetroWest Region Parent/Caregiver Survey on Youth Substance Use

The purpose of this survey is to collect information from
parents/caregivers around their opinions of youth substance
use in your community. We will use this data to determine
priorities, raise awareness, and support local programs and
policies to address youth substance use. We hope to aid
parents and caregivers like yourself so that you are best
able to support youth in healthy and informed decision
making.


This survey is anonymous and should only take about 6
minutes of your time. If available, we encourage one or more
parents/caregivers from your household to complete this
survey. As a reminder, every question is optional and your
participation is completely voluntary.
